Disease History is a Therapist task in Escape from Tarkov that sends you to Reserve to collect two medical records from locked rooms on the second floor of the hospital (the White Bishop building). One record is behind the RB-SMP door, lying on the floor between the shelves. The other is behind the RB-KSM door, at the bottom of the shelving or cabinet on your left. Bring both keys, extract with the records, and hand them to Therapist.
The hard part isn’t the room search. It’s getting the two keys and surviving the hospital stairs with them.

Where to find the Disease History medical records
Both records are on the hospital’s second floor, a short walk apart. Fight your way up the stairs first, since the building gets traffic, then open the two rooms one after the other.
- Head to the White Bishop hospital on the east side of Reserve. Sportskeeda and GGRecon both use the White Bishop logo and the nearby helicopter as the landmark.
- Take the stairs up to the second floor and clear the hallway before you start using keys.
- Turn left at the top. The RB-SMP door is the second door on that side; the Japanese EFT wiki describes it as the middle of three doors.
- Open it with the RB-SMP key. The record is on the floor between the shelves on the right after you walk in.
- Leave the room and go back right along the hallway. At the end, the RB-KSM door sits to the right of some planks; the Japanese wiki notes a red plate on it.
- Open it with the RB-KSM key. The record is low down on the left: the EFT Wiki puts it in the bottom-right compartment of the left-hand cabinet.
- Extract, then hand both records to Therapist.
Older guides don’t agree on which record is “#1” and which is “#2”. The wiki pairs #1 with RB-KSM, while GGRecon and Sportskeeda pair it with RB-SMP. Don’t let that confuse you: the room locations above are consistent across all of them, and you need both records either way.
How to get the RB-KSM and RB-SMP keys
Buy them on the Flea Market or loot them. GGRecon notes neither key can be bartered from a trader, and that Scavs can carry them in jackets, pockets and bags.
Prices swing a lot. As of October 9, 2026, Tarkov Market showed the lowest RB-KSM listing at about 85,500 roubles and the lowest RB-SMP listing at about 165,000 roubles. Tarkov Forge‘s averages were higher, around 93,000 and 219,000. Check both before you buy.
Each key has 40 uses, and Tarkov Forge lists Disease History as the only task that needs them. That’s why selling the keys back after you finish is common advice.
When does Disease History show up?
For years the task needed player level 15, and GameMaps lists Pharmacist as the task before it. Patch 1.1.0 (Kord Breach, August 3, 2026) changed how side tasks appear: they now open in batches of two to four as your trader loyalty level rises, according to Insider Gaming’s patch summary. tarkov.help currently lists level 15 plus Therapist loyalty level 3. If it isn’t on Therapist’s task list yet, raise his loyalty level first.
Disease History rewards
The long-standing reward was 7,200 EXP, +0.03 Therapist reputation and 30,000 roubles, plus medical items (a Grizzly medical kit, Ibuprofen and five CALOK-B in older guides). Patch 1.1.0 rebalanced side task payouts toward money and experience, and the trackers haven’t settled: tarkov.help still shows 7,200 EXP and 30,000 roubles with an Ophthalmoscope, while Tarkov Forge lists 28,000 EXP. Trust the reward panel in Therapist’s Tasks tab over any website, as of October 2026.
The task has long been on the list for the Kappa container. 1.1.0 also reworked the route to Kappa, so check the in-game requirement list if that’s your goal.
Mistakes that waste a run
- Picking up a record and dying. The wiki is clear that a record you don’t extract with is lost. Going back means opening the door again, which costs another key use.
- Bringing keys in your backpack. Sportskeeda suggests storing both keys in your secure container so a death doesn’t cost you the keys too.
- Rushing the stairs. Sportskeeda’s advice is to clear the building floor by floor on the way up, before you stop to turn a key.
- Trying to do both rooms at once when the raid is going badly. Records can be handed in separately, so grab one and leave if you have to.
- Buying the keys before the task is active. The records don’t spawn without it, so check the task is on your list before spending six figures.
Frequently asked questions
Can you hand in the two medical records in separate raids?
Yes. Each record is its own hand-over objective, so you can grab one, extract, turn it in to Therapist, and come back for the other later.
Do the medical records spawn if you don’t have the quest?
No. Both records only appear while Disease History is active, so there’s nothing to find in those rooms beforehand.
What happens to a medical record if you die?
You lose it. The record goes into your quest item inventory and is gone if you fail to extract, so you have to go back for it.
Are the RB-KSM and RB-SMP keys used for anything else?
Disease History is the only task the trackers list for either key. Each key has 40 uses, so many players sell them on the Flea Market once the task is done.
Which medical record is in which room?
Guides number them differently. What matters is that one record is in the RB-SMP room and the other is in the RB-KSM room, and you need both.
